References and Supporting Documentation

References

Candidates must provide the required professional and client references using NARM’s current forms and submission instructions. References must be completed by individuals who can provide relevant information about the candidate’s professional conduct, clinical preparation, or care.

References must be submitted directly to NARM by the person providing the reference unless NARM’s current instructions specify another method. References submitted by the candidate will  not be accepted.

Clinical Documentation

Candidates must submit complete documentation of the clinical experience required for each PEP phase. Documentation must:

  • identify the candidate and supervising preceptor;
  • accurately describe the date, setting, clinical role, and care provided;
  • be signed by the NARM Registered Preceptor;
  • be consistent across all related forms and records; and
  • meet NARM’s requirements for supervision, sequencing, timing, and verification.

Each NARM Registered Preceptor must verify only the clinical experience and skills completed under that preceptor’s direct supervision.

Supporting Records

NARM may require candidates to submit additional records to support or clarify information provided in the application. Supporting records may include:

  • clinical logs;
  • education records;
  • preceptor verification forms;
  • credentials or licenses;
  • certificates of course completion;
  • practice documents;
  • signed student–preceptor agreements; and
  • other documentation relevant to determining eligibility.

Accuracy and Verification

All information submitted to NARM must be complete and accurate. NARM may contact preceptors, reference providers, educational programs, credentialing organizations, licensing authorities, or other appropriate sources to verify submitted information.

NARM may request clarification, corrections, or additional documentation before determining that a requirement has been met. Documentation that is incomplete, inconsistent, unverifiable, or not submitted in the required form may be returned for  corrections,  subject to audit or rejected in accordance with the PEP Audit Policy or not accepted.

Candidates are responsible for retaining copies of all submitted materials and for ensuring that required third-party documentation is received by NARM.

See Audits and Verification for additional information.
